An immunoassay system to investigate epidemiology of Rocahepevirus ratti (rat hepatitis E virus) infection in humans

Background & AimsRat hepatitis E virus (Rocahepevirus ratti; HEV-C1) is an emerging cause of that divergent from conventional human-infecting HEV variants (Paslahepevirus balayani; HEV-A). Validated serological assays for HEV-C1 are lacking. We aimed to develop a parallel enzymatic immunoassay (EIA) system identifies individuals with exposure. also conduct the first seroprevalence study in humans using this validated EIA system.MethodsExpressed HEV-A (HEV-A4 p239) and (HEV-C1 p241) peptides were characterized. Blood samples simultaneously tested HEV-A4 p239 p241 IgG EIAs. An optical density (OD) cutoff-based interpretation algorithm identifying seropositive or was RT-PCR-positive infection sera. This used measure 599 solid organ transplant (SOT) recipients age-matched immunocompetent individuals.ResultsBoth formed virus-like particles. When run EIAs, distinct clusters minimal overlap 2D plot OD values. The final showed high agreement RT-PCR results (Cohen’s κ = 0.959) able differentiate sera accuracy 94.2% (95% CI: 85.8 – 98.4%). 7/599 (1.2%) among SOT 4/599 (0.7%) individuals. 5/11 (45.5%) these patients had history transient unknown cause.ConclusionsHEV-C1 exposure identified 11/1198 (0.92%) Hong Kong indicating endemic estimate humans. valuable tool investigating epidemiology risk factors infection.Lay summaryRat has recently been discovered infect humans, but antibody tests lacking making it difficult gauge how common is. developed test can identify past rat rates found approximately 1% all people exposed previously.
